Return to running: your "how to" guide

Decide when an injured runner can run, establish their current tolerance and build a clear, graded plan that restores distance, pace and confidence without repeated flare-ups.

Should your patient run through mild symptoms, replace running with cross-training or stop completely? Stopping running can reduce fitness and confidence, and your patient may not need or be prepared to do this. However, running too far, fast or progressing too quickly can flare their symptoms and set back their recovery.

Bone stress injuries (BSIs) can resemble muscle injury or overload, hip flexor pain and other misdiagnoses, especially in their early stages. Missing a BSI can lead to delayed healing, recurrence or serious consequences for your patients. Which clinical clues matter, when is imaging required and what needs to change beyond simply resting the painful bone?

In this module, you'll explore the bone stress continuum, clinical assessment and how to identify low and high-risk bone stress injuries, and when to include multidisciplinary management.

ITBS and patellofemoral pain

Your patient wants to run, but knee pain keeps limiting their training. Patellofemoral pain (PFP) and iliotibial band syndrome (ITBS) are two of the most common running injuries, and commonly settle, then come back with a vengeance when patients restart or progress their running.

How can you confidently diagnose and identify the cause of your runner's knee pain? What exercises will settle your patient's pain and build their running tolerance? How can you progress your patient's exercises and guide them on running distance, speed and hills, without aggravating symptoms?

In this module you'll explore how to use the four pillars of running rehabilitation to effectively treat ITBS and PFP. You'll develop clinical reasoning that takes into account your athlete's pathology, pain, training load, gait and priorities to develop a practical treatment plan.

You'll discover how to
  • Differentiate PFP and ITBS from other sources of knee pain, so you can begin treatment with an accurate diagnosis.
  • Understand why stretching or massaging the ITB is unlikely to solve the problem, so time is redirected towards more useful strategies.
  • Modify downhill running, camber and training volume to settle ITBS while avoiding complete rest.
  • Adjust knee flexion, quadriceps demand and hip adduction during exercises and running to alter patellofemoral joint load and create a tolerable starting point.
  • Use education and shared decision-making to improve confidence and adherence, so your athlete becomes an active partner in their rehabilitation.
  • Build strength, control and power through progressive exercise, so capacity catches up with running demands.
  • Use step rate, stride width and other gait options to unload sensitive tissues while the runner stays active.
  • Restore distance, hills and speed, so your patient returns to full training with fewer flare-ups.

Running Repairs started at the finish. To be precise, at the finish of the Brighton marathon around a decade ago. I was watching in the Spring sunshine unaware that this was the beginning of my work and my life changing forever.

As I watched the runners battle through that finishing stretch I was in awe. I cheered so much I was hoarse for days! I've always been a runner and have competed since primary school cross-country but I'd never tackled the 'big one'. That was about to change. Within 24 hours I'd signed up for my first marathon, a race that ignited my passion for running rehab.

As I began training for my marathon, I pushed myself and was constantly keeping niggles at bay. I went on a runner's forum to see how others were getting on and what I found shocked me.

There were so many runners managing injuries and all were struggling to find quality treatment. I searched online and everywhere I looked it was the same - poor advice, "must-do" stretches and foam rolling, multiple different opinions without any evidence behind them.

I wanted to help all these runners, and I became a man on a mission! I spent hours answering question after question on running forums but with more questions coming up than I could answer I soon realised I needed to create a home for quality, evidence-based advice on running injuries - running-physio.com.

I explored strength and conditioning, load management, training optimisation and psychosocial factors in injury, gaining knowledge and experience all the time. The more my knowledge and skills developed around the latest research, the more I was able to help runners.

My treatment results with injured athletes started to get noticed and I became the 'go to guy' for running injuries, seeing everyone from weekend warriors to international athletes.

As my knowledge and passion grew I wanted to help fellow therapists get great results with athletes. I took what I learned and carefully shaped it into the first Running Repairs course which I presented in July 2014, covering all the key principles in managing running injury as well as how to treat all common pathologies we see in runners.

I've been very fortunate since then to present the course all over the world and meet some incredible therapists. Their feedback has taken Running Repairs to a new level.

In stepped Clinical Edge legend and good friend of mine, David Pope with the answer! David suggested we develop an online course where we could include ALL the aspects I wanted to cover, and therapists all over the world could enjoy it at their leisure. We combined my content with his extensive experience in online learning, and Running Repairs Online was born.

Since opening the Running Repairs Online course for enrolment in July 2017 we've been blown away by how popular it is and delighted with the feedback from participants. It's been amazing to see a community of thousands of Running Repairs Online course graduates grow around the world.

The Running Repairs Online course contains more than 50 hours of CPD as well as links to research articles and other resources. The Running Repairs Online is accredited by the Faculty of Sport and Exercise Medicine UK who have awarded it 26 Continual Professional Development points, and has also been approved for 30 CEU credits with the Arizona Physical Therapy Association.

Who can enrol

Anyone with a Health Science degree (or a student currently enrolled in a health science degree). The education contained on the site has a certain level of expected knowledge, so you must be a Physiotherapist, Osteopath, Chiropractor, Sports Doctor or Exercise Sports Scientist. Massage therapists and remedial therapists are unable to become members or join the Running Repairs Online course.
